Mandatory - Items

• Prescribed form (Form I) used for the application.
• Form I, being submitted in Duplicate.
• Please use Prescribed Form for the Application.
• Please fill the Application form completely and correctly.
• The amount paid as fees correct.
• Copy of RC book from RTO for all the no. of vehicles mentioned in point 6 of the Form 1 attached.
• Partnership deed / MoA attached.
• Whether the original registeration certificate of the undertaking enclosed.
