Every such application shall be accompanied by relevant physical documents as listed below.

Mandatory - Items

• If the undertaking is not operating then: is the application being submitted within not less 30 days of the date on which applicant proposes to operate
• If the undertaking is already operating then, is the undertaking applying within 60 days of the date of commencement
• The undertaking employing workers >= 5
• Prescribed form (Form I) used for the application
• Form I being submitted in Duplicate
• Are both forms completely filled
• Are both forms filled correctly
• Challan/fee receipt attached
• The amount paid as fees correct
• Copy of RC book from RTO for all the no. of vehicles mentioned in point 6 of the Form 1 attached
• Partnership deed / MoA attached.
