• Submission of application along with,
  • Original certificate in form IV, with detail of supplied plate nos furnished on overleaf.
  • Photo copy of this certificate with endorsement of supplied plate nos, with suppliers (in whose name the certificate is issued) stamp & signature.
• Scrutiny by technical section and acceptance by Director, on certificate.
• Delivery of original & accepted endorsed copy of certificate to applicant.
